Finding Hope and Healing, LLC is committed to protecting the privacy and confidentiality of your health information. We are required by law to protect your protected health information (PHI), provide you with this notice, and follow the privacy practices described here.

Your Rights

You have the right to:

  • Ask us to correct information you believe is incorrect or incomplete.

  • Choose someone legally authorized to act on your behalf.

  • File a complaint if you believe your privacy rights have been violated.

You will not be retaliated against or denied care for exercising your privacy rights or filing a complaint.

How We May Use or Share Your Information

We may use or disclose your health information as permitted or required by law, including for:

Treatment — to provide, coordinate, or manage your care and, when appropriate, communicate with other professionals involved in your treatment.

Payment — to bill you, your insurance company, or another responsible payer for services provided.

Health Care Operations — to operate the practice, maintain records, improve services, and conduct appropriate administrative and clinical activities.

Mental health and substance use information may be subject to additional protections under federal or state law. When additional protections apply, Finding Hope and Healing, LLC will follow those requirements.

Except as permitted or required by law, we will not use or disclose your health information without your written authorization. You may revoke an authorization in writing as permitted by law.

Our Responsibilities

We are required to maintain the privacy and security of your protected health information. We will notify you as required by law if a breach occurs that may have compromised the privacy or security of your information.

We may change this Notice of Privacy Practices. Any revised notice will apply to the health information we maintain and will be made available upon request and on our website.
